Exclusions & Limitations

Can not be used with another dental plan, dental insurance or financing.
For referrals to specialists. This plan is only honored at QC South Dentistry, not any other dental office.
Patient’s portion of the bill is due the day of service.
Non refundable premiums.

No roll overs if not used within the 12 month period.
Invisalign treatment not included.
Ultra low dose 3D X-Ray not included- recommended every 5 years, starting at age 6 or if needed for diagnosis.
Scaling and root planning is considered treatment, so will be 20% off in the Perio patient plan.
