Limitations
and Clarifications

  • Can not be used with another dental plan, dental insurance or financing.
  • For referrals to specialists. This plan is only honored at QC South Dentistry, not any other dental office.
  • Patient’s portion of the bill is due the day of service.
  • Non refundable premiums.
  • No roll overs if not used within the 12 month period.
  • 5% off annual fee with each additional family member.
  • 10% off Invisalign and Whitening Treatment
  • 20% off Ultra low dose 3D X-Ray- recommended every 5 years, starting at age 6 or if needed for diagnosis.
  • 20% off Scaling and root planning.
